Stage 1: End-User Needs Assessment
This stage is the cornerstone of the entire closed loop model; the objective is to capture challenges, needs and expectations of CCS Heart Failure Consensus Conference end users defined as:
- Specialists
- General practitioners
- Pharmacists
- Nurses
- Policy-makers
- Patients
- And any others with direct vested interest in implementation of these guidelines
Although this is identified at Stage 1, it is important to note that all of these stakeholders provide focused input and feedback to the Heart Failure Consensus Conference Program throughout all stages of the closed loop model. It is this feedback that allows the Primay Panel to determine the ‘how to’ to closing the gap between ‘what we know and what we do’. |
